ACTIVE EMPLOYEES' ELIGIBILITY FOR BENEFITS

Eligibility of Hourly Active Employees

Eligibility Requirements.

If you are an Active Employee of an employer required to pay hourly contributions to the Trust, you will be eligible for benefits on the first day of the second month following the work month in which you have accrued 300 hours or more in covered employment for which contributions must be paid to the Trust. The 300 hours must be accrued during no more than 12 consecutive work months.

EXAMPLE

New Employee

July--------100 hours
August------75 hours
September- 60 hours
October-----65 hours
equals------300 hours
-------------125 hours Deduction for December coverage
-------------175 hours Hour Bank Reserve in December

Employee has accumulated' 300 hours with hours worked July - October.
Lag Month is November. Employee is eligible for benefits on the first day of December.

NOTE: If employee would have worked a total of 520 hours as another example during the same period above, the hour bank reserve would be at 395 hours in December after deduction for December coverage.

Previously Eligible Employees

December Hour Bank Reserve---------------------175 hours
November Hours reported in December----------100 hours
--------------------------------------------------------275 hours
--------------------------------------------------------125 hours
Hour Bank Reserve January------------------------150 hours

NOTE: The employee became eligible December and had a bank of 175 hours. November hours are received by Administrator in December and posted to employee's bank for eligibility month of January; 125 hours were deducted for January coverage resulting in a reserve bank of 150 hours.

Reserve Hours Account

1. Subject to the provisions set forth below, a Reserve Hours Account shall be established for you composed of hours worked in excess of 125 hours for which contributions are made, or required to be made, on your behalf by one or more contributing employers during any work month. In no event shall the number of hours in your Reserve Hours Account exceed 500 hours.
