Extension of Eligibility During Disability

If you are an Active Employee who becomes temporarily disabled while eligible for benefits under this Trust, you will be entitled to an extension of eligibility for a maximum of 12 months, if the following conditions are satisfied:

1. all hours in your Reserve Account, if any, have been used; and
2. you are available for immediate employment after recovery from the disability, by an employer contributing to the Trust.

The extension of eligibility shall not extend beyond three months after your recovery from the disability.

Labor Dispute Continuation

If your employer is required by a collective bargaining agreement to pay all or part of the cost of your Trust contributions under the group policy and you stop work due to a labor dispute, you may continue your insurance during the labor dispute by the rules below:

1. This continuation will be allowed only if:
(a) you make a payment each month for your insurance in the way and amount specified below;
(b) such payments are collected from at least 75% of the employees who stop work due to the labor dispute; and
(c) timely payment of the premiums for the insurance are made to the prepaid medical plan (e.g., to Kaiser or Health Net).

2. You must make your monthly payments on each premium due date to the Trust.

4. This continuation will end on the earlier of: (a) the date you start active full-time work with an employer other than the employer you stop working for due to the labor dispute; and (b) the last day of the sixth month that follows the date you stop working.
